Atividade eletromiográfica, dor, incapacidade e perceção de melhoria na cervicalgia crónica após fisioterapia multimodal

Introdução: Poucos estudos exploraram a evolução clínica em doentes com dor cervical crónica. Compreender o seu curso clínico pode ajudar os médicos a avaliar os resultados individuais, a definir objetivos e a estabelecer um prognóstico.
Objectivo: Descrever a evolução clínica dos doentes com dor cervical crónica inespecífica (CICP) em resposta a um programa de fisioterapia multimodal.
Metodologia: Um total de 15 doentes com CICD participaram num programa de fisioterapia multimodal de seis semanas. As avaliações iniciais incluíram características sociodemográficas e história clínica de dor cervical. Os principais desfechos foram a atividade muscular cervical, a dor, a incapacidade e a perceção global de melhoria. Para a análise estatística foram utilizados os testes de Friedman e post hoc com correção de Bonferroni. Resultados: Todas as variáveis estudadas variaram significativamente (p<0,001) ao longo do programa. Nas primeiras 3 semanas, a atividade muscular do esternocleidomastoideo e do escaleno anterior diminuiu significativamente 64% e 24%, respetivamente. Foram também observadas melhorias clínicas precoces em todas as variáveis auto-referidas, com uma redução da dor e da incapacidade de 50% e 24% no final da terceira semana de intervenção.
Discussão: Os doentes com DCIC submetidos a um programa de fisioterapia multimodal que inclua exercícios específicos de controlo motor podem experimentar uma influência a curto prazo na atividade dos músculos superficiais do pescoço e na redução dos sintomas.
Conclusões: Os resultados deste estudo podem ser úteis para o desenho de ensaios clínicos que determinem se as melhorias se devem ao tratamento ou à recuperação natural da doença.
