Efeito da terapia cognitivo-comportamental combinada com treino físico em adolescentes com sintomas de depressão elevados: um ensaio clínico randomizado controlado

Resumo

Objectivo: Investigar os efeitos da terapia cognitivo-condutual (TCC) combinada com exercício regular versus intermitente na depressão e na qualidade do sono em adolescentes.

Participantes e métodos: Os adolescentes dos 12 aos 17 anos com depressão moderada estão equitativamente distribuídos por três grupos, cada grupo de 20 doentes. O grupo recebeu uma sessão semanal de TCC de 60 minutos combinada com exercício regular de 60 minutos, três vezes por semana, durante 12 semanas; O grupo B recebeu TCC semanal de 60 minutos mais ejercicio intermitente de 60 minutos uma vez por semana durante 12 semanas e o Grupo C recebeu TCC semanal de 60 minutos a solo durante 12 semanas. Os sintomas depressivos dos participantes foram avaliados antes e depois das intervenções de 12 semanas através da Escala de Depressão do Centro de Estudos Epidemiológicos (CES-D), a qualidade do sono foi avaliada antes e depois das intervenções de 12 semanas através do Índice de Qualidade do Sono de Pittsburgh (PSQI).

Resultados: Após as intervenções de 12 semanas, o grupo A apresentou reduções significativamente menores nas pontuações de sintomas depressivos CES-D em comparação com o grupo B e o grupo C. A qualidade do sono melhorou significativamente nos três grupos, e o grupo A registou o maior aumento. A comparação entre os grupos B e C não apresentou resultados significativos. A avaliação estatística intragrupo não mostrou diferenças significativas entre os grupos.

Conclusão: Entre os adolescentes deprimidos, o exercício regular de três semanas combinado com TCC conduziu a uma diminuição significativamente maior dos sintomas depressivos e uma melhor qualidade do sono que o exercício intermitente uma vez na semana mais TCC ou TCC sola. Estes hallazgos indicam que a frequência do exercício pode ser chave para potenciar os benefícios antidepressivos do TCC.
