Efeito da libertação suboccipital versus mobilização de tecidos moles assistida por instrumentos na cefaleia de tensão: um ensaio clínico randomizado controlado

Resumo

Enquadramento: A cefaleia de tensão é a forma mais comum de cefaleia primária associada ao aumento da tensão muscular. Este estudo teve como objetivo comparar os efeitos da libertação suboccipital e da mobilização de tecidos moles assistida por instrumentos (IASTM) na cefaleia de tensão.

Métodos: Cinquenta e seis mulheres com idades compreendidas entre os 20 e os 40 anos, com cefaleia de tensão e índice de massa corporal (IMC) entre 18,5 e 24,9, foram aleatoriamente alocadas a três grupos. O Grupo A (controlo) (n=18) recebeu fisioterapia convencional, incluindo compressas quentes e um programa de exercício. O Grupo B (experimental) (n=19) recebeu libertação suboccipital para além da fisioterapia convencional, e o Grupo C (experimental) (n=19) recebeu IASTM para além da fisioterapia convencional. As medidas de resultados incluíram a intensidade da dor avaliada pela Escala Numérica de Avaliação da Dor, o limiar de dor à pressão medido com um algómetro de pressão, o impacto da cefaleia avaliado pelo Teste de Impacto da Cefaleia, a incapacidade funcional cervical avaliada pelo Índice de Incapacidade Cervical e a frequência da cefaleia medida pelo número de dias com cefaleia por semana. As medições foram registadas no início do estudo e após quatro semanas de tratamento.

Resultados: A MANOVA mostrou que os Grupos B e C apresentaram resultados significativamente melhores do que o Grupo A em todas as medidas (p < 0,001). Não foram encontradas diferenças significativas entre os Grupos B e C na intensidade da dor, no limiar da dor à pressão, no impacto da cefaleia ou na frequência da cefaleia. No entanto, o Grupo C apresentou uma maior melhoria da capacidade funcional cervical em comparação com o Grupo B (p = 0,001).

Conclusão: Tanto a libertação suboccipital como a IASTM estiveram associadas a melhorias na intensidade da dor, no limiar da dor à pressão, no impacto da cefaleia e na frequência da cefaleia em mulheres com cefaleia de tensão. A IASTM demonstrou uma maior melhoria da função cervical.
